Higüey's Basilica Cathedral of Our Lady of Altagracia, consecrated in 1971 after nearly two decades of construction, is the most important Catholic pilgrimage site in the Dominican Republic. Its modern concrete arch, designed by French architects, was built to house a small 15th-century painting of the Virgin of Altagracia, the country's patron saint. Every January 21, the feast day draws large numbers of pilgrims from across the country and the wider Caribbean, some arriving on foot from distant towns.

Devotion to the Virgin of Altagracia predates the current basilica by centuries. The small devotional painting itself is believed to date to the late 15th or early 16th century, brought by early Spanish settlers, and the image was formally crowned by the Catholic Church in 1922. Pope John Paul II visited the shrine in January 1979, part of his first international trip as pope, reinforcing Higüey's status as the country's principal site of Marian devotion.

Outside the basilica, the city retains a colonial-era core, though little remains from Higüey's founding in the early 16th century, one of the older Spanish settlements on the island, since most historic structures were replaced over the centuries.

Higüey as a Gateway to the Punta Cana Coast

Because La Altagracia province stretches from Higüey's inland farmland to the all-inclusive resorts of Punta Cana and Bávaro, the city functions as a service and administrative hub for the coast, home to the roads, markets and government offices that support the resort economy nearby. Sugarcane fields and cattle ranches still cover much of the surrounding plain, giving travelers who pass through Higüey a glimpse of a more rural, working side of the region before returning to the resort strip.

Visitors exploring the wider Dominican Republic sometimes add Higüey onto an itinerary that also covers Santo Domingo's colonial zone or the beaches farther along the coast, treating the basilica visit as a half-day stop rather than a standalone destination.
